What happened

On September 30, Tennessee attempted to execute death-row prisoner Christa Pike at Riverbend Maximum Security Institution in Nashville using pentobarbital. Officials administered the protocol's initial dose and a second dose, but Pike remained alive. She was taken by ambulance to a hospital, where her attorneys said on October 2 that she remained in critical condition receiving life-saving care.

Why it matters

Tennessee's Department of Correction said the written protocol permits no further procedure beyond the steps carried out that night. Governor Bill Lee suspended the state's remaining executions for 2026 and ordered a third-party review. The review had not yet established why the drugs failed to kill Pike, so the medical cause remains unresolved.
